A separate form is required for each delegate. Follow the steps as indicated on the form and mail the form with payment to:

Pre-registration forms must be received by February 11, 2009. Payment must also accompany your pre-registration. No refunds will be issued after February 3, 2009, and a $25 processing fee will apply.

Follow the steps as indicated on the form and mail the form with a $50 deposit (per delegate staying in room) to:

The Potawatomi Inn will send confirmation to all delegates making reservations. Reserve only the nights needed as all reservations are guaranteed and delegates will be charged for all rooms and nights reserved unless written cancellations are received no later than February 13, 2009. Full conference reservations are processed before partial conference reservations. Check in time is after 4:00 p.m. Visa, Master Card, or American Express is acceptable for payment at checkout.
A $50 deposit (per delegate) should be mailed with the meals and lodging form and is required to hold your room. The deposit payment must be made by check, money order, or credit card.
